For general radiology exams, no appointment necessary.

You must have in hand the original request signed by your doctor and your valid health insurance card.

Monday to Thursday : between 8h30 & 19h00
Friday: between 8h30 & 16h30

To take an appointment, you can :
Call 514-365-7766 or 514-368-ECHO (368-3246) Monday to Thursday : between 8h30 & 19h00 Friday : between 8h30 & 16h30	For information concerning your exam.
Come to the clinic: Monday to Thursday : between 8h30 & 19h00 Friday : between 8h30 & 16h30	Bring the original request signed by your doctor.
Fax both sides of your exam request to 514-365-8622 OR for an MRI exam, fax your exam request and, if available, the questionnaire completed and signed by you and by your doctor to 514-365-8622	Make sure that your name and phone numbers (home and work) are clearly indicated. We will contact you within 48 hours following the reception of your request to schedule an appointment with you as soon as possible. If available, also send us the preliminary questionnaire for an MRI, signed by you and by your doctor.

Attention: CSST and SAAQ cases
We must ask for an examination authorization before scheduling the appointment
Send us your doctor’s request, along with your coordinates,your CSST or SAAQ file number with the name of the agent taking care of your case and his/her Fax number.

Important : When arriving for your appointment, make sure you have:

• the original request signed by your doctor.
• your valid health insurance card and the method of payment for the exam, if needed.
• The images on CD and copies of results of previous exams related to this exam.

Please arrive about 15 minutes early to open your file.
